Yes, using wildcard rules. A prefix pattern like US:* or a suffix pattern like *RAW matches every channel that fits, and the rule keeps applying automatically as your playlist refreshes, so you only set it up once. Exclude patterns hide matches; prioritize patterns float them to the top instead.

With a subscription, yes. Hidden channels, favorites, renames, custom categories, ordering and your matching rules sync through your iCloud account to every iPhone, iPad, Mac and Apple TV signed in with it. Without one, everything still works and stays on the device you set it on.
